Ipamorelin is a synthetic pentapeptide and selective Growth Hormone Secretagogue (GHS) that mimics ghrelin by binding GHSR-1a receptors in the anterior pituitary. In a landmark study by Raun et al. at Novo Nordisk, ipamorelin was identified as the most selective GH secretagogue tested — triggering robust GH release with zero measurable effect on cortisol, ACTH, or prolactin. This selectivity profile distinguishes it from all other GHRPs studied to date.

In vitro and preclinical mechanistic observations
Binds ghrelin receptors in the anterior pituitary to trigger pulsatile, physiologically-timed GH release that mirrors natural young-adult secretion patterns.
Unlike GHRP-2 and GHRP-6, ipamorelin does not activate the HPA axis — producing no cortisol, ACTH, or prolactin elevation even at high doses.
Induces pulsatile (not continuous) GH release, preserving natural GH rhythms and avoiding the receptor desensitisation seen with continuous GH secretagogues.
GH released by ipamorelin signals hepatic IGF-1 production, driving downstream anabolic, recovery-promoting, and metabolic effects.
